When you need hreflang
If you serve the same content to more than one language or country (en-GB vs en-US, or a translated site), you need hreflang. A single-language UK site can skip it — we mark that as not applicable, not a fail.
The usual bugs
Invalid language codes, no x-default, and no self-referencing tag. All three are common in plugins that generate hreflang from incomplete locale lists.
